Skilled Nursing Facility Limited Visitation Protocols (07/30/2020)

Dear Family, Friends and Residents,

We are happy to notify you that our facility has met all the CMS, CDC, and DOH guidelines to begin limited visitation.  Among other requirements, our facility has developed a NY Forward Safety Plan and have been COVID-19 free for 28 days.

At this time outdoor visitation only will begin Wednesday, July 29, 2020.  If you wish to visit your loved one you must call ahead (preferably 24 hours in advance).  Appointments will be made through our Recreation Department only.  Please do not call the receptionist or unit as they will not be able to schedule a visitation appointment for you, and will make every effort to connect you with the appropriate department.  You may contact a Recreation Staff Member at (914) 350-9010 x 119 to schedule your visit. Please keep in mind visitation may have to be re-scheduled due to poor weather conditions.
